81 PITFALLS IN LATERAL ACCESSORY PATHWAY ABLATION: THE IMPORTANCE TO RECOGNIZE MITRAL ISTHMUS CONDUCTION BLOCK DURING REDO PROCEDURES

Abstract Introduction Inaccurate mapping and ablation of left-lateral concealed AP can result in mitral isthmus conduction block, mimicking concentric retrograde activation. AVRT recurrences are frequent because leaves the through intact. Case Report A 50-years-old patient with history previous ablations underwent third redo procedure due to reoccurrence SVT. Quadripolar catheters were placed His bundle RV apex; a decapolar catheter was CS. Retrograde during ventricular pacing non decremental. CS activation AVRT. Mitral anulus mapped via transeptal approach, showing justifying “concentric” atrial appearance, left counterclockwise circuit short VA interval distally site. Single RF application 1.9 sec interrupted conduction. Effective confirmed by failure induce tachycardia induction AV dissociation using adenosine pacing. Conclusion Change pattern reoccurrences may be referred iatrogenic block achieved ablation, rather than presence concomitant misdiagnosed AP.
